Passports

Passport applications must be completed at the time of the appointment, or your appointment will be rescheduled. In consideration of other appointments scheduled, we ask that you be on time for your appointment, or we may have to reschedule your appointment to another day. Appointments are available Monday through Friday 8:30 a.m. - 3:30 p.m.

Passport Forms

To apply for a U.S. passport for the first time or to renew a passport by mail, applications can be downloaded from the Passport Agency's website at www.travel.state.gov or are available at this office. Forms should be filled out completely using black ink. Do not use white out to correct mistakes.

Basic Requirements

All applicants must appear in person. Please bring your completed passport application (DS-11) to your appointment. Do not sign the application before your appointment each signature must be witnessed by a passport acceptance agent.

Proof of Citizenship

Original documents must be submitted photocopies will not be accepted. Your original documents will be returned to you by the U.S. Department of State. You will need one of the following:

Photo Identification (Children under 18 can use parent's ID)

Identification must bear both the signature and the photo of the applicant. Identification must not be expired. The most common forms of ID are:
